Building Collaborative Pathways for School Counselors and Transition Coordinators

This presentation explores the critical role of school counselors in the transition planning process for students with disabilities and emphasizes the power of collaboration between school counselors and transition coordinators. Participants will examine how aligning the course of study with academic and career plans supports student outcomes, identify strategies for fostering effective partnerships, and discuss practical tools to tailor transition services for individual student needs.

Learning Objectives:

  • Describe the complementary roles of school counselors and transition coordinators.
  • Apply Kohler’s Taxonomy for Transition Programming 2.0 and the American School Counselor Association (ASCA) National Model. 
  • Align each student’s Course of Study (CoS) with their Academic and Career Plan (ACP). 
  • Identify methods for ongoing collaboration between school counselors and transition coordinators.
  • Identify a data-driven action plan that incorporates family engagement strategies, interagency collaboration, and student voice.

Bios:

Kelly Ligon, M.Ed., is a Research and Demonstration Associate for the Center on Transition Innovations at Virginia Commonwealth University. She has been in the field of special education for over 30 years. Since 2000, she has provided training and technical assistance to educators across Virginia who support students with disabilities. Her passion is preparing students for life after high school.

Dr. Patricia Uricchio Morgan is the Coordinator for Transition and Planning Services at the Virginia Department of Education. She brings extensive experience in special education, professional development, and improving outcomes for students with disabilities. Her expertise includes curriculum alignment, instructional strategy support, and data-driven improvement. Dr. Morgan has led professional learning sessions on transition instruction, Evidence-Based Practices, High-Leverage Practices, and Specially Designed Instruction. She has served in roles such as Coordinator for GMU’s TTAC, Transition Specialist, and Director of School Counseling, with a strong focus on career development, post-secondary planning, and community partnerships.
